The following required registration forms for the 2019 season can be found in the documents section of Team App. Feel free to print and fill out the forms prior to attending a registration event.

In addition to the forms above the following documents will also need to be turned in:
1. Birth Certificate – Bring the original along w/ a copy. We will verify the copy w/ the original and keep the copy on file. The original will be given back to you.
2. Final Report Card from the 2018-2019 School Year – This is required for participants ages 7-15 and can be turned in once the school year has ended and final report cards have been sent home.
Organization Mandatory Raffle Fundraiser
All participants are required to sell 60 raffle tickets at $2 per ticket. You can elect to not participate and pay the 2019 Opt Out " fee of $100.00. This fee can be paid online as well and is only available at initial registration only.
Tackle Football Registration
Tackle Football Ages 5-11
Registration Fee is $ 185.00 per participant
Sibling Discounts are available.
**NEW THIS YEAR** With a $100 deposit by July 31, you can:
1. Secure your child's JERSEY # (first come first serve basis)
2. Secure a spot on one of our tackle football teams.
Registration fee includes: Game jersey, game socks and the usage of a helmet, shoulder pads and integrated game pants.
